On behalf of the University of Hartford athletic training staff, we would like to welcome you back for another season. We hope you had a great summer. All returning student-athletes must fill out our (1) student-athlete health history questionnaire, (2) medical authorization (parts A-D), (3) health insurance information.
Please print out the forms and take the time to fill it out with as much information as possible. After completing the forms, please mail it with a copy of your insurance card (both front and back) to the address listed below. The information that you provide will be vital for your future care for the upcoming athletic season.

If you have sustained an injury while away from Hartford, please do not hesitate to call or email us. We will be happy to discuss the injury and help to provide you with the best care that Hartford has to offer. Also, if you have seen a physician for an injury/illness over the summer, or had an MRI, X-ray or CT scan, please bring along the images for our physicians to review.
