Dear Hartford Hawk:

 

On behalf of the University of Hartford athletic training staff, we would like to congratulate you on becoming a Hawk. We hope you are looking forward to the exciting challenge of college athletics.

 

As part of our screening process, all freshmen and transfer student-athletes must fill out our (1) student-athlete health history questionnaire, (2) pre-participation exam (completed by  your physician), (3) medical authorization (parts A-D), (4) health insurance information and (5) Sickle Cell waiver. The information that you provide will be vital for your future care. It is extremely important for us to know any past injury, illness, and/or problems which you may have sustained. Please print out all of the forms and take the time to fill them out with as much detail as possible, in order to make a smooth transition into Hartford Athletics.

Due to new NCAA rules, any student-athlete taking medication for ADHD (Attention Deficit Hyperactive Disorder) needs to have documentation of testing and treatment on file with us. If you have been tested, diagnosed and are taking or have been prescribed medication for ADHD we will need documentation of the testing, physicians notes and prescriptions when you arrive on campus. 

Starting in 2010-11, the NCAA has passed new legislation for sickle cell testing. For more information and to fill out the required sickle cell testing form, click here.
